Care and Case Management for Older People in IrelandSarah Delaney, Rebecca Garavan, Hannah McGee and Aodán Tynan Health Services Research Centre The Council recommends that future national, regional and local policies on health and social care service provision for older people be developed to embrace a Care Management approach to service co-ordination and planning at a management level, and Case Management as the means by which health and social care services are tailored and delivered to vulnerable older people who are on the margin between living at home and living in long-term care.
