Details An opportunity to hear the important lessons learned from the Practicalities and Possibilities programme, talk directly to people who have been making changes, and to be part of themed practical sessions.

The conference will launch the new Practicalities and Possibilities materials capturing these lessons and examples of ‘what works’ for older people, services and organisations.
Workshops will include

Introducing person centred thinking with older people.
Best practice in support planning with older people.
What ‘co-production’ with older people really means.
Taking a strategic approach to person centred thinking and ageing.
Delivering personalisation with older people.
Person centred approaches and mental health in later life (including dementia).
Person centred thinking for health and health care services.
