Details: It is estimated that 30% of the UK population are living with a long term condition. With an ageing population and increasing incidence of long term conditions the challenge to health and social care is set to continue. Currently, it is estimated this challenge equates to 80% of NHS costs and two-thirds of NHS activity. Managing Long Term Conditions is the forum to support this challenge. 48 sessions from 6 conference streams, eight free seminars; fringe programme and an exhibition village.
