Aging at Home: Connecting Research and Clinical Care - Toronto, Canada
At this conference, attendees will be updated on recent studies in applied research that are advancing understanding of how best to address the challenges faced by older adults with chronic disease. Issues explored will included: How can the healthcare system assist in the self-management of chronic disease? What are the most effective pharmacologic and non-pharmacologic approaches to treatment? What can be done to enhance functioning in seniors living in the community so that they can age at home and maintain a good quality of life? The conference includes morning lectures led by internationally recognized leaders in health care research and chronic disease. The afternoon workshops led by scientists and scientist-clinicians focus on specific issues related to the management of chronic disease by older adults and their caregivers.
